DLA 4 page form.

Got the new 4 page form for DLA today.
Quite surprised its come so early as its not due for renewal until february 2011.

I also know that they looked at my ESA medical results last time i was given it and have not had a recall for my ESA medical yet that is up for renewal in December.

Anything i need to do about it as i know i have seen a few negative posts on here about the 4 page renewal form.

Hi,

If you do a forum search there is a wealth of information about this form including how others have dealt with the form and the results.

Just because the form is 4 pages long does not limit you to 4 pages in response. As usual try to send in as much evidence as you can.
