You are not normally able to get free prescriptions while you receive Contribution Based ESA, but can if your receive Income Related ESA.
The forms you have received are for you to switch from CB to IR. You should seek face to face advice before switching, as ESA(IR) is means tested and this may affect the amount of money you recieve.
Dave and I posted alternatives that may allow you to receive free of reeduced cost prescriptions.

If you look at the post 5 days ago in this forum entitled 'Exemption from prescription costs' gordon has given a link which will give you more information about pre payment & exemption certificates. Anyone can apply for a pre payment certificate irrespective of income & how its paid; for the exemption certificate you have to meet one of the qualifying criteria & your GP has to sign the form & send it off.

What what I do without you people your fantastic thank you so much for your help and understanding I do try and seek the answers elsewhere but as you know its not easy but you can rest assured the people here really do understand.
