When do i need a sick note and which to which JP

I have recieved the letter from DWP saying i have failed the assessment.I was on incapacity and had this assessment to migrate onto ESA.

1. The decision letter from the DWP was not from my local jobcente plus.It was from one thats over 30 miles away.Is this normal?

2.In this letter it states that my benefit/s will stop on[*a date in august*].Do i need a sick note from my GP from this date in august because i am appealing or do i need the sick note from the date on the decision letter?

3.When i get my sick note,do i send it to my local jobcentre plus who have always dealt with my benfits(even though its a few years since i spoke to anyone there)OR do i send it to the jobcentre plus thats over 30 miles away?

ps.I have already appealed and sent the appeal form off to the jobcentre plus on the decision letter(over 30 miles away).But does this mean everything,including any sick notes have to go there also?Is the sick note called a med3?

1) Yes, this is normal and this is the address to which all correspondance should be sent.

2) Yes, that is the date from which you must be covered, there is no harm in starting the 'fit note' (sicknote, med 3) from an earlier date if that is more convenient for you.

3) See 1.

I sent my fit note in to the JP a week before the date that it was needed to so i could get the assessment rate until my appeals been sorted.Today,i get a letter from JP saying i need to send a fit note in to get the assessment rate and if i don't before a certain date they won't pay me the assessment rate.I have copies of fit note,proof of posting,and proof of delivery :S

Hi J

If the "fit note" that you posted to the DWP covers you from the date that your IB ceased, and is not yet due for renewal, I would contact them by phone informing them of this.

I would then repost copies of the "fit note" proof of posting, and the proof of delivery, with a covering letter of complaint.

Thanks bro,i have already posted a renewel fit note too,that i have proof of as well.I shall send them the copies like you advise.

Now i am confused,After getting the "you still need to send a fit note in before we pay you the assessment rate" letter yesterday,today i received another letter from a different JP saying they ARE paying me the assessment rate from the date my IB stopped.Do i still go ahead with sending copies etc that you advised last night in above post or not?Also,while i'm on the assessment rate can they ask me to go to work focused interviews?
