It does happen, not often. I'm afraid there is no magic formula to getting a paper based assessment.

I had a home medical and got a copy of the medical report. I then received the 'We have all the info we need' letter from the DWP.

However, Atos have sent since my GP a report form to fill in.

Does this mean that Atos will now generate a new medical report, and if so, would I need to ask for any particular form/document number in order to get a copy?

It is unusual for a GP report to be requested after the file has been returned to DWP. If this is what has happened, I cannot explain it.

I suppose it is possible that the GP report was requested before the file was returned to DWP and, for whatever reason, the assessor did not wait for it to arrive. If this is the case, I would not expect the GP report to have much/any affect on the decision (but might be very useful at appeal!).

Are NHS hospital consultants obliged to fill in and return a PIP medical report form if they receive one? How long would Atos wait if the form isn't sent back?

I believe GP's are the only ones under an obligation to reply, but that does not mean that your Consultants won't if contacted. ATOS will probably give then a couple of weeks at most to reply.
