Hi, more advice please.
1) Is it ok to use doctors' letters that have been emailed to me as evidence, once printed off seem just like a copy of an original letter.
2)The doctor has my christian name, surname , date of birth and address correct on the letter but there is a mistake in that my title is Miss and the letter refers to me as Mrs throughout, do you think that ok

It should not be a problem, most surgeries use headed paper so even if you got a copy of your medical notes then the GP's addressing information would still be missing.

1) I think I am filling in the form sufficiently without adding my own extra sheets. But then I wonder if that means I have less of a chance of succeeding gaining an award. ???
2) I have 2 letters from different health professionals saying why I would not be able to attend a medical assessment centre . One of these letters doubles up as evidence for diagnosis so will they use that letter for both, should I send 2 copies so that i can be used both to explain why I cannot get to an assessment centre AND be used as evidence confirming diagnosis?

There's no need to send two copies just refer to the letter twice :)

Thank you. I am rather lost with how to do the form really. Do I need to keep referring to the evidence throughout the form or can I just send the evidence in with the form.

Also, one of my conditions has been going on for years with many hospital letters etc 15 years or more ago. Then brief references now and then over the years since in summarised doctors notes. Current statement of fact letter includes this condition but only as a two word diagnosis. I really do not know what to send. Surely I cannot send in mountains of medical records but then is the current statement of fact of two word diagnosis sufficient. I kind of feel they may need evidence of the long term nature of it. Do applicants send in vast number of pages of their medical histories?
