For Reg 29 you also need to explain why the risk is mitigated by any prescribed medicine or reasonable adjustments that an employer might make.

I have mentioned about when after my accident in work that I went back to work doing a light duties job 9 to 5 but the company had to let me go because I was in pain and I was dropping things and I could not concentrate on my job due to my lack of sleep with pain.My condition have got a lot worst since then and I am on stronger medication.

That's a good point you made about travelling to and from work I didn't think of that.

Do i mention reg 29 /35 on the form or do I say special circumstances and do I mention this was one of the reasons I was put in a support group and do I enclosed the decision notice from the tribunal in 2013.

I was also thinking of enclosing with my form a report that had from my specialist when I first had my accident and another one from 2013 that mentions about my condition and how it would effect my working life and two doctor s letters from 2013 plus the one up to date letter if I can get it back in time.

Well I sent my esa50 today I didn't have a up to date letter from my gp I said on the form that it will follow still waiting for my doctor secretary to finish it she said it will be ready by the end of the week.my esa50 had to be sent by wednesday of this week so I sent it by recorded post next day delivery.

I sent a old medical report by my specialist when I first saw her and two letter from her dated 2012 and 2013 two letters from my gp dated 2013 and a letter from my pain clinic dated 2014 saying about infusions.

My condition has got a lot worse since then and I have pain in other joints so I could have done with the up to date letter from my GP.

My wife did a apendix with all that I enclosed with the esa50 and numbered them with my national number and name my wife also enclosed the tribunal decision notice and mentioned how rules 29 and 35 applied to me in 2013 and how my symptoms have got worse since then.
I enclosed prescriptions and information on side effects and also meds that I have been on in the past.

Do you think I have done everything right so far in what I have sent them.

So now I will play the waiting game and see what happens fingers crossed and all that.

Thank you for all the advice and links I will let you know how I get on one more thing the post cost me £7.25 thought I would let others know.
