Thanks Gordon,

All of my admittances are for a chronic illness, the medical intervention and treatments simply allow me to get back to the point where I can manage at home again. The disability affects me across a number of the activities so I may be writing the same things in more than one activity.

Hi mods. Happy New Year to you all.

I'm again trying to do little bits at a time on this form in readiness for it landing through the letterbox.

Can I check with you guys: on the old style form or PIP2, there's a question for listing your health conditions and disabilities and the dates when they started. On the AR1 I can't find a question for this information. Q3a & b ask about treatments and medications but nothing about listing your conditions. Should I list these at the end in Q13- is there anything else we should know? Or do DWP not want this for a re assessment ? How will they know what your disabilities are to start with though if we don't put the information down somewhere?

The AR1 form is an update form intended to be used in conjunction with your last PIP2, so you would only need to list your conditions if there has been a change to them since you completed the last PIP2.

Since my last assessment I have unfortunately been diagnosed with a few new conditions. Also do I take that the assessors will now be reading my first PIP claim and so be aware of what has gone before.

As I only have a few new conditions would you suggest I just note these at Q13.

Yes add them to Q13 but if they have an impact on your ability to complete the PIP activities then you also need to documents this.

Hi all.

I'm finding myself getting v confused over the AR1 form, might be to do with all my medications.
I've only completed one PIP form and read that if you receive the PIP2 you should complete it as if it was a new claim and assume the DWP/assessors no nothing about you as they will not have looked at any previous information.

So when/if I receive the new AR1 form will they have access or looked at the information on my first claim and the assessors PA4 report. I'm just trying to sort out what information about my disabilities I need to provide on an AR1.
