this could be a good thing as my GP asked me to do this for some insurance forms ect and we sat together filling in the forms it gives you an opportunity to have your point of view direct to your GP as he completes the docs and you can clarify any grey areas there and then

Thanks for the tips folks.My doctor is normally very good and quite sympathetic but this completely new to me so I dont know
what to expect. Are those 70 odd questions likely to be something your own doctor will ask ,or would those just be relevant in the case of a DLA appeal?

The doc has been and gone and I am not filled with hope. Just one question are they not supposed to read back what they have written to you and you sign it?

Hi,

That used to be the practice but no longer. You are nowadays not asked to sign the form.
