PIP Assessment result

Is it possible to ask for the.....I am sorry can`t remember the code......How the decision was reached on a PIP assessment that took place 3 years ago.
Reassessment is about due and it would be handy to have the details from original decision.

Gordon what is the name of the form I am asking for please I can`t remember the code that you normally tell people to ask for.

It`s OK thank you I was meaning you normally say to people apply for ESA 85 or something like that when they are wanting to know how the PIP decision was reached.
