Hello,
Last week i received a set of forms requesting information re parttime/voluntary work? my daughter or partner usually deals with mail of all kinds as memor/ycoping/dealing isnt dealt with and my morphine and daily meds doenst help.completely forgot to contact you to ask is this routine or usual or trend or,am asking now while remeber as each day different,daughter due back from hols but we just thought best ask if anything to be aware of as none of questions apply or have have applied for a long time and she will countersign when back to post it back

You don't mention the benefits that you are receiving so it's difficult to give advice, if this is for ESA then it would be in regard to your doing Voluntary or Permitted Work, but as you say that you are not doing either of these it should be no problem.

Sadly due to memeory situation i have only just remembered that i placed a question on forum,this is just meds and morphine im afraid,im on,ESA and PIP is what i receive,awarded in 2013,reassesed Sep 2016,havent worked since illness forced basic redundancy about 2011/12 and condition has as expected and told by consultants deteroirated,did attempt to go back to well paid work approx 2014 but sadly only lasted 2 days,dwp were told of this so we are unsure whether to put this down as last time i worked or my real previous job sick redundancy date in 2011/12 as do not wish to complicate life any further or unneccesarily however fully aware of the way these admin things are fraught with peril,have not worked and incapable regrettably of ever working again and limited life expectancy after 10 hrs emergency surgery in 2011 so part time or voluntary totally unapplicable but partner and daughter have a little trepidation as to just which date to put down for last time actually worked

I have no sight of the form that you have been asked to complete so I can only advice you to complete the questions as asked, if you are unsure what to put then include both dates you mention in your post with an explanation, you can always attach this as a letter to the form.

It isjust a Jobcentre headed letter requesting information regads my part time or volunatary work and 4 forms (6784/0915?)asking when last worked,wage slips etc so that is what is puzzling my daughter partner and i,surely cant be referring back to 2014 when my partner recalls a deduction was made at end of that year for the days work that appeared via tax return (as the work was through an agency) i attempted to return to work as it was a very good job and felt obliged to try but ultimately was impossible and attempt was aborted as made worse,they do all post and suchlike as i simply dont open it or deal so i wont recall,we shall add a note as you suggest probably

Just been reminded thye were informed of attempt at time via letter via welfare rights too just to keep things above board but as i say it was some 3 years ago? so still puzzled as to why letter has come out of the blue
