Is there PIP blank form on here? was created by capainFAIRY
I have been reading through the posts and now cannot find the one that had a link to a downloadable PIP form, can you help me locate it please?

No, I'm afraid there is no downloadable PIP form. Every form issued will be printed with a unique barcode that identifies the claimant.

You can't print it off or fill it in online, but a sample form is available if you want to see the layout/questions on gov.uk here .

I must stress again though that you must not print it off and fill it in, because each form is barcoded to link to your claim, and the sample obviously doesn't have that, so it is for information purposes only, but quite useful so you can understand the format of the questions for when the real form inevitably drops on your doormat.
