We deal with ESA, this includes Contribution Based ESA, but we cannot give financial advice on the forum, although we can usually explain the rules.

Other members are free to reply to posts if they wish, but we tend not to have conversations of the forum because most members usually ask a question in the forum, get an answer and then do not return until they have another query.

Only subscribed members can post in the forum, although it can be read by the "public".

New to this. Does your local job centre see your ESA 1 claim form and personal details such as medical condition that are on the form? I believe you can download the form after filling it in and take it to your local job centre. Otherwise I believe you can claim and do the form over the phone, is this a local job centre and I understand if you do the over the phone ESA 1 claim they send you a statement to sign. Does this statement have to go to the local job centre.

Hi C,

i will merge this post with your other ongoing ESA topic.

Please keep all future ESA related queries to this topic, by replying to the topic.

Yes, you can either ring the phone number and fill The ESA 1 Claim Form in over the phone, or you can access a PDF on-line version where you can either type your answers in, then print it off and post to the correct address, or print it off blank, fill it in with a pen, then post it off to the correct address.

You will see that there is a red box at the end of the form that you click to give you the address to return it to :

See : Here where there is a link to the download for the PDF ESA 1 form.

You should obviously send your fit note with your claim form and keep copies of everything :

Sending Documents to the DWP, ATOS, Capita or Maximus.

You should Bookmark/Favourite this page in your Internet Browser now. That way you can return to this topic easily to pose any further queries that you may have by replying to the topic.

See : This Topic also !

Thanks in anticipation of your future co-operation in keeping all your ESA related queries to this topic ! :)

If I make an ESA claim, I know it is to my benefit to list all medical problems, but one of my problems I do not want to include in the ESA claim because it is something I cannot write or talk about. I believe my other medical problems will be more than enough to support an ESA claim.
Can I ask my GP to put on my fit note all my problems except for the one I don t want to talk or write about. I mean can I ask my GP to omit that one or do I just have to let the GP do the fit note as he thinks fit (excuse the pun).

Hi Cruella,

Again, I have merged your query with the rest of your ESA topic. Please see Gordon's reply and do not start a new topic every time you think of another question!

It doesn't really matter what is on the fitnote, it certainly does not need to list all of your conditions.
