It comes under the same heading as the Med-3, if the DWP used it in the making of the Decision then they should have included it in the bundle, if they did not then it would not be there, but you are entitled to submit it and I would highlight the date that it was completed and returned.

Send it tomorrow and take 6 copies with you on the day and ask the Clerk of the Court to confirm it was submitted, if not ask them to distribute the copies.

It's what the DM used that counts, however, it implies that it should have been in the file that the assessor returned to the DWP, but as the Med-3 was not supportive it's probably something to stay away from.
