When I have time I am going to merge your queries into one topic. It helps us enormously when everything relating to the same claim is kept in one place.
Don't forget that the issue is not whether you would be able to accomplish someone on the rare good day. Reliability is the key word. This means -
• Safely – in a fashion that is unlikely to cause harm to themselves or to another person.
• To a necessary and appropriate standard – given the nature of the activity.
• Repeatedly – as often as is reasonably required.
• In a timely manner – in a reasonable time period.

I have recently completed the PIP2 form with assistance from someone at AgeUK. I prepared all of the further info for each question on typed sheets in Word using the B&W guide. The helper said that they were very good. My writing is completely illegible and he wrote what was necessary on the form. If you know you need help I would look for it.

Did you not take a copy of your ESA50 before you submitted it? You really need to keep a copy of your PIP2!
If your ESA assessment was more than six months ago then your information is likely to be offsite, so if it was more recent, contact the DWP office that dealt with your ESA claim, if longer then you will have to issue a Subject Access Request

The assessment report will be an ESA85 if you had a face to face and an ESA85A if you did not.
