Pip Application Form was created by zaib
I received sms from DWP sent the form and that I need to return it before the date givent. I am worried how to to fill in / supporting evidence it is very hard for me to concentrate . I have many health problems ,I am being seen by neurologists for the last 5 years for my debilitating Trigeminal nerve pain it starts from my left eye spreading to left side of head it is triggered by concentrating , reading ,I can not read , I can not watch TV . it can also be triggered by noise, lights , weather changes , so unpreditable I can do nothing with the pain /headaches. I have very dry eyes need drops every 2 hours every day yet cannot open eyes in morning before being helped to wash them and start my day , I have a multi vessel heart disease diagnosed 2015, artheritis in both hands back , shoulders , many problems weak muscles weak bones over the years had a surgical premature menopause at age 33 . I do need help for many daily activities cannot travel alone because of anxiety , loss of concentration etc . I visited age concern for advice on what to do bogged down by a sickening noise from a huge construction started in front of our privately rented home making me virtually homeless all day as I can not return home in this crippling noise, if I have to -
But they gave me a number to call / request a PIP form!!! it is on its way now . I cannot read any lengthy guides shall have an attack of Nerve pain , my husband does the reading for me but he is very depressed on thoughts of applying disability ! , it looks very difficult may be only disgusting and most ppl dont get it in the end ?

Welcome to the forum, you might want to have a look at the following FAQ which explain where everything is

Welcome to Benefits and Work

I've moved your post to it's own topic, we ask members not to post their questions in other people's topics, it's not fair on the original poster and it can lead to the wrong advice being given or taken.

I would start by having a look at our PIP Claim guide. See

www.benefitsandwork.co.uk/help-for-claimants/pip

If you have questions on what you have read then reply to this post and we will do our best to help.
