It is not impossible (double negative) to get am Enhanced Mobility award due solely to mental health issues, but recent Upper Tier Tribunals would make it very difficult, the PIP Claim guide explains the issues.
I'm afraid there is no electronic form for PIP, each PIP2 issued has a unique bar code, however, you can attach additional pages prepared in a word processor, just make sure that each has your full name and NIN and that they are numbered "page x of y". For each question mark on the form "see page Z" and on the additional pages "continued from question Q".

You can request a paper version of the PIP1 but I have to warn you that other members who have done this have had problems with the form being sent in a timely manner, the competed PIP1 needs to be back with the DWP by the same date as the call has to be made be.
If you do make a written request then you must manage the process carefully to ensure that you do not make the cut off.
If you can get a relative or friend to phone on your behalf then you will avoid a lot of worry.
