The person needs to see the child on a regular basis and to have knowledge of the problems that they have.

The letter needs to state who they are, how they know the child and how often they see them and what problems they have seen. Finally they need to attest that the information they have provided is correct to the best of their knowledge.

Whatever they can attest to reasonably. so if they have knowledge of how he walks because they have seen him have problems then they could include this, but if he has problems sleeping and there only knowledge is from you telling them then they should not make any comment on this.
