Missed PIP medical, reason "not good enough".

I missed my review assessment due to an epileptic seizure (I forgot due to memory loss), and my PIP was stopped. They said they sent a letter out to inform me, but it didn't arrive, so after 2 weeks from the date of this mysterious letter, I asked for another one. It has the original date of the first letter, but I was away from home when it arrived. It was dated the 15th September and said I need to request a reconsideration within "1 month", but I only came home on Monday 17th.
I have a very basic template for my response, but could use some advice on how to phrase the personal details.

Have you previously told them that you an a Fit on the day of the assessment?

Have a look at the PIP MR & Appeal guide on the following link it will explain how to request an MR, there are also template letters that you can use in the PIP section.

I've looked at the MR & Appeal guides, and I didn't see any advice about missed assessments, only appealing decisions after the assessment. Have I missed it in those guides?

The process is no different, you are still disagreeing with a Decision, your reason is that you had Good Cause for missing the appointment.

I assume you have missed two assessment appointments? Your post suggests that you missed the first appointment only! If you missed the first appointment then you can ask why you havn't received a second assessment appointment.

If you've missed two assessment appointments then the onus is on you to provide evidence or reasonable cause as to why you've missed the second appointment.

The official DWP line on the missing (letter)post is the DWP only have to prove the date the (DWP)post was sent!However,the claimant (you)when dealing with the DWP always has to provide evidence that (your)post was sent and received by the DWP.

As for being away when the post arrived. You will need to look up the latest guides on informing the DWP of absence from your postal address.

This has happened several times recently. It seems that the assessment provider is returning files to DWP if the claimant fails to attend, rather than cancelling ahead of time.

I believe the office are looking into this at the moment.
