DLA to PIP Application - Correct Form

Hello, I have to transfer from DLA to PIP and completed the telephone stage over a week ago. Today I got the form to complete - How Your Disability Affects You, as expected. However as transferring from DLA to PIP (and based on your advice guide) I was expecting this to be called PIP2. The one I received is PIP.1003. They are very similar. Is it ok to go ahead and use the PIP.1003? Many thanks for your help.

Hi Sandhead and Welcome to the forum, you might want to have a look at the following FAQ which explain where everything is

Each PIP form is marked with a unique barcode that identifies the claimant so it is particularly important that you return the one you were sent!

Thank you for the reply - I have been a member for many years, used the site, forum and guides. As per your guide I was expecting PIP2 not PIP.1003 - appreciate the one they sent is unique to me - just wanted to check if THEY had sent the correct form. Thank you.

The problem is that the DWP refer to the forms by their given names (PIP2 and AR1) rather than the form reference that is actually printed on the documents, and to make matter worse the form reference number does not actually appear on the sample forms that the DWP provide.

I too received my PIP.1003 form today. After looking through it I realised this number didn't refer to any of the forms I'd looked through on B&W website. Most worryingly of all was question 3 - on preparing and cooking a meal. It wasn't there in format I'd prepared for. For example the one that applies to myself "unable to prepare or cook a meal at all" isn't there and the two questions that do have tick boxes don't actually refer to my abilities. I've sat worrying about it for the rest of the day, too worried to check thro' the other questions in case they're even more horrifying and wondering what to do next. The DWP are putting in "useful" guides which bear little resemblance to the form and confuse matters more. I'm not a stupid person by far but this form has stumped me.
