The questions being asked are for different things. So as some examples; using an aid does not mean that you need assistance and needing prompting does not mean that you need to use and aid. Of course you may need both but for different reasons, so you might need prompting to prepare and cook a meal due to depression and an aid to help you complete it due to a physical impairment.

You need to answer each question as relevant your limitations, either by using the tick boxes or in the comments section below.

For question 6 you are saying that you can only wash with the use of the bath seat, Any rituals that you have with your washing would be something you should detail in comments box or additional sheets.

Thanks Gordon I'm still perplexed as to whether I answer both sets of questions in q6 I need a person to bathe me but I use a bath seat so do I tick yes in both a and b boxes or just yes in box b and explain in info box ?? So do I leave no boxes unanswered ? Sorry to be a pain but I feel I'm saying I can do something with an aid when I can't many thanks

Thanks Mrs H I just feel by ticking box a then b I'm contradicting myself like I don't use an aid then saying I need physical assistance for same question can I tippex out or just leave box ticked and explain at end of form ? Thank you

Hi Mrs H question on section 3 I cannot prepare or cook my own meals so ticked I do not use aid or appliance to cook meal in section a and yes I need help from another person in section b should I have left section a blank as it seems I'm saying I can cook a meal ??

OK let's try this again!

The questions are independent of each other and all of them must be answered.

You need to read and answer the question that is being asked, at the moment you appear to be second guessing your answer to one based on your answer to another one.

Let's take Preparing Food. The numbering is mine to signify how you should approach the questions.

1. If you can cook a meal without problem then answer "no" to (a) and (b) and say you can prepare and cook a meal without problem in the comments box.

2. If you can't cook a meal without some form of help then or you can't prepare cook a meal at all then;

3. Can you cook a meal with the use of suitable aids?

If you can't cook a meal, even with the use of aids then you should answer "no".
If can cook a meal when using an aid then answer "yes"
If sometimes you can and sometimes you can't then answer "sometimes"

In all cases explain the comments box why you have answered as you have and in the case of "sometimes" how often you are unable to.

4. Can you cook a meal with the help of someone else. It does not matter what you answered to (a)

If you can't prepare and cook a meal even with prompting, assistance or supervision then answer "no"
If you can prepare a meal if you receive one or more of prompting, assistance or supervision then answer "yes"
If you sometimes need prompting, assistance or supervision and sometimes don't then answer "sometimes"

In all cases explain the comments box why you have answered as you have and in the case of "sometimes" how often you are unable to.

The approach is the same for the other activities.

So for Washing and Bathing you have said that you need to use a seat (yes to aid) and that you need have assistance to wash (yes to someone else helping). You then need to explain why you need the seat and why you need someone to wash you.

Sorry Gordon but I cannot prepare or cook any food for myself and I answered yes ? Do they prepare all your food to 3b you say it's no ? Does prepare all my food not mean cooking it ? Sorry to be such a pain I can't get my head round it you have the patience of a saint I'm finding this very stressful thanks
