Dear Mods
I have been given the 27th June for an assessment following submission/renewal of esa 50 0217 form. Can I request in writing to wait in a separate room on arrival at the centre as I am absolutely unable to sit in a waiting room full of other people. Who do I write to? Can I also request for the assessment to be recorded and if so who do I write to?

All of this has caused a deteriation in my MH issues and some. Trying to see my GP before assessment. Plus I have no one to accompany me as I know that is a positive from my last assessment in 2013.

Absolutely dreading and frightened about the day already .

Contact the assessment venue with both requests, there is a template letter in the ESA section for requesting the assessment is recorded.

Make sure you follow up the requests a couple of days later.

Dear Mods

Apologies for not replying sooner but I'm all over the place with severe depression, intense fear and trepidation about my ESA assessment next Tuesday.

When I try to access the esa members guides for template letters for recording face to face assessments and template letter to request a separate waiting room, I keep getting logged out of B&W. My subs is up to date and paid.
I get the message below.

sorted tech issue I think.

don't know how I'm managing to write this. Saw my GP this Wednesday just gone and long story short after she read the letter I sent to surgery before appointment and going through issues etc including my upcoming esa reassessment and my planning suicide, she immediately arranged for the community mental health crisis team to come and visit me at home for assessment - hospital or at home.

CPN's been visiting me at home all week and will be over the weekend. Monday or Tuesday they are trying to setup appointment to see one of the team's psychiatrists and to review my medication etc.

Tuesday 27th June should have been my esa reassessment date, due to sending request to assessment centre to ask for individual side waiting room and everything recorded.
Letter back from them explained they do not have facilities for a side waiting room.

they say in letter they have cancelled 27th June appointment and changed to
Tuesday 18th July. Original appointment time was 2.15pm now its changed to
9am. Said its first slot of the day.

Short sentence in letter said hope this helps. Also said no need to arrive 10 mins early as well. But numerous jsa people signing on etc will be in the ground floor building and state of DWP and delays etc lends itself to no guarantee will arrive and straight into room for reassessment.

before this crisis, I wasn't seeing psychiatrist regularly as in middle of 2015, the 2 people I was seeing and universally hated by everybody and not just service users, decided I should be discharged and just carry on with repeat medication from gp every month.

so since exhausting me completing esa 50 02 17 form and no supporting docs from gp, mental health workers etc, condition has severely deteriated. Plus one of team visiting me has identified I have Speech Dyspraxia. I understand they are not a speech therapist but Verbal communication affects me in everyday situations in different scenarios.

so how do I address no waiting room, not guarantee being seen immediately at 9am could be delayed , change of assessment date/time, suicidal planning and immediate referral to community mental health crisis team, current intensive daily home treatment visits from community mental health team and review of medication and MH team arranging appointment wit Psychiatrist. As well as identification of Speech Dyspraxia.

All of which I couldn't put down on esa form sent to me for reassessment purposes.

so sorry for long questions.

You need to concentrate on getting yourself in to a stable, safe state of mind, but if you can then put your recent problems down on paper and send a copy to Maximus and the DWP office dealing with your claim, it may make a difference to your requiring an assessment,

Is your GP going to help you with your claim now?
