Requesting an extension for ESA form submission

Hello,

I would like to request an extension in my ESA form submission, which I did last time I applied for ESA in Autumn 2015 (I am in the support group), but I was just wondering whether I might be asked to attend an assessment before I have submitted my form if I do this?

I am on ESA for my mental health conditions and under the CMHT but the Clinical Psychologist who I had appointments with until last year left the CMHT and I am unsure who to list as my main medical professional on the form. I see a social worker at the CMHT, had appointments with a CBT therapist until December and but I am also on the waiting list for my Clinical Psychology sessions. I have been unable to visit my GP for a couple of years but am considering writing to him to ask for a letter of support.

I have less than a fortnight until my form needs to be submitted- what should I do?

There is no extension available for returning the ESA50, even if you are told by the DWP that you can have one, unless you have the extension in writing then you should stick to the original date.

If you cannot complete the form on time then my recommendation is that you return it as it is and send in the rest of the information as soon as you can.

Are you making use of our ESA Claim guides?

Try and simplify what you are doing. Start with a brief explanation of your problems for each activity and then go back and provide more detail.

Hi Gordon,

Thank you for your reply and all of your help- I've only just remembered that I posted this message.

I now have a letter of support from my Social Worker and she has referred the assessor to my last medical letter from my previous Clinical Psychologist so that is helpful. I wrote a letter to my GP but have been informed by the medical practice (who intercepted the letter) that it will take three weeks for them to write a letter and cost £25 so that's not really an option. I don't know whether to see if one of my parents can phone the GP practice on Monday and ask them to write a medical note or whether my social worker's letter will suffice? She has basically said that my condition remains much the same as it was two years ago, if a little worse as I've had to move back in with my parents due to my condition. Will this be ok in itself?

Hi Gordon,

Thanks so much for your help.

I'm a bit concerned- I do have a letter from my Mental Heath Social worker but my Mum requested a GP letter and was told it would take 3 weeks and cost £25. I wish I had realised earlier because that's not an option now.

My social worker did refer to the letter that my Clinical Psychologist wrote in 2015 and stated that nothing had changed. To your knowledge does it need to be a medical letter rather than a Social Worker letter to qualify without a medical assessment (I did not have to attend last time)?
I've misplaced a letter from my 3 month stay in hospital in 2012-13 and think that it is too old now anyway.

In 2015 I was granted an extension for my form- did they just stop allowing extensions? Thanks!

You don't need to repeat your questions :) all posts are moderated and will not appear in the forum until approved and the Moderators are not usually on over the weekend.

I'm not sure what you mean by a note from your GP, if you mean a Fit Note then this won't add anything to your claim.

We have seen call centre staff offering extension to claimants in the past, but they have no authority to do this.

If you have moved address then it is essential that you update the DWP and Maximus of where you are now or letters will continue to go to your old address including any invite to an assessment.
