Yes you can take some notes with you, if you have to refer to them then explain why you are doing so.
Your carer needs to be very cautious in prompting you, the assessor wants to hear from you, a good assessor should allow some input, but if they ask your carer to be silent then they should take the request seriously, or you risk the assessment being curtailed.
