Medical Report Received - it's a work of fiction!

Sorry Gordon, I've addressed this in my own Getting Out thread but I've reread the report and ruminated and now I'm furious and I need to do something. I've dropped my claim for ESA so theoretically I could just let it go but I already feel as if I'm letting the side down by cancelling the claim; I really feel the need to flag this up.

I was in the ESA Support Group for mental health problems and moved to WRAG following a medical assessment in March. I asked for a copy of the report which arrived today and there is so much that is wrong with it in the mental health section that it's almost laughable; in fact it contradicts itself many times. And it's really difficult to read - what does "Significant functional disability seems likely in adapting to change and coping socially at this time" actually mean? But it's not the mental health part which has upset me, it's the section about my phyisical health, which I wasn't even claiming for.

Under Medical Examination Findings in the General section it states

"The details and scope of the physical examination were explained to the client, including advice not to perform any movements causing pain or discomfort. The client gave consent for the process to proceed."

It then goes on to report how well my limbs did under examnination which were apparently as follows:

Lower Back
Forward flexion to: mid shin
Squat and rise: Is Full

Left Leg
Hip flexion is 130 degrees (normal)
Knee flexion is 120 degrees (normal)
Knee extension is full
External hip rotation 45 degrees (normal)
Power Normal
Ankle Plantarflexion Normal (50 degrees)
Ankle dorsiflexion Normal (20 degrees)
Straight leg raising is Normal (more than 70 degrees)

It then goes on to report the measurements for my right leg, and then my upper limbs. It also states that I was able to get on to the couch for assistance and that I did not appear to have any difficulty using a step to get onto the couch.

It also reports that my skin was not cyanosed, that there was no finger clubbing and that there were no swollen neck glands.

The thing is - I did not give consent for the process to proceed because it just did not happen. There was no physical examination of any kind, nor any mention of one. In fact, I can't remember seeing a couch in the room. She stayed on her side of the desk and we stayed on ours yet she was able to pronounce that I had no swollen glands?! She stated that twice during the examination I was able to stand up unaided from my chair but the only time I stood up was when we were finished. Once, not twice.

In my case, the decision is moot but how on earth are they able to just make stuff up and put it on a report which is a fundamental part of the decision making process? It's mind-boggling but it's also just wrong!

I feel the need to let somebody know about it, but as I'm not actually appealing, who do I write to?

In the past I have never requested copies of medical reports. Now I cannot recommend strongly enough that anybody who has a medical assessment ensures that they ask for a copy of ghe report to be sent to them. I dread to think how many decisions have been made on the basis of non-factual reports.
