Hello,
I received my PIP reassessment form today, having sent my ESA-50 for reassessment 11 days ago.

I have a few questions:
1. Is there an online/printable version of the reassessment form?

2. It says on the form not to include evidence that is more than two years old, however for my ESA claim I included letters from a three-month long hospital stay at a specialist hospital in 2013/14 and would like to include these- will these necessary be disregarded? It seems silly, if a condition has stayed the same for 5 years to ignore older evidence.

3. Is it possible to request an extension for PIP or is the situation the same as ESA (i.e. someone may (over the phone) say that you can be permitted an extension, but then this does not count).

4. My Social Worker recently wrote a supporting letter for my ESA application but could this be used to PIP too? In the letter she said that my condition has remained the same as it was when my Clinical Psychologist wrote an in-depth report in October 2015 and I included a copy of my Psychologist's report in my claim. The report discusses my problems with preparing food, washing/showering, toilet issues, decision making, social interaction, coping with change and mobility- so some of the descriptors. The report is obviously older but my Social Worker has stated that it still applies so could they use this as evidence (also it is 19 months since the letter, not 2 years).

5. I cannot attend a GP appointment but my Mum has offered to visit the GP for me and hand in copies of the evidence I have from the mental health team and she has offered to request a letter of support from the GP (it is not the Practice policy to support benefit applications but my GP might write one if asked in person). Would simply requesting a letter that states that my health conditions remain the same help my case?/be useful?

6. I think that while my daily living problems remain unchanged (I scored 36 points for Daily Living last time), my mobility is actually more restricted (I scored 4 points last time so no award for Mobility) however I think that the Mobility component is very difficult to get even at Standard rate? would it be best just to tick 'No change'? Does ticking 'No change' and including in-depth answers to each section make a reassessment Medical any less likely?

1. There is not an electronic version that you can complete on your computer.

2.It's your claim, you can include whatever evidence you believe to be relevant.

3. As far as I am aware an extension should be available for completing the AR1 as it is for the PIP2.

4. There's no reason why you cannot include these.

5. It's better than nothing, but as always more detailed information would be better.

6. I really can't offer any advice on this without knowing what you scored the four points for and how your conditions have changed, but if you don't mention the change it would seem very unlikely that you would an award for Mobility.

Thanks for the great help/answers Gordon!

I do not have any physical conditions (multiple and chronic psychiatric conditions, starting in childhood) so I imagine a Mobility award is unlikely?

Last time (in 2014) I was awarded 4 points for needing help to plan a journey. For the last 16 months I have not left the house apart from to go for walks in the countryside with family members (never alone). I cannot cope walking past people along pavement and actually panic if we see anyone on the countryside walks!! I don't walk in town or go to the shops or anything like that (my parents buy food and toiletries). However I can run alone provided it's in the countryside but only because I feel safer running as I can get away from my anxiety triggers quickly. After a dog walk with a family member or a run by myself I have to perform extensive rituals.
I think that stating that I run meant that I got no award, but maybe that's fair? I am not too concerned about getting a Mobility award but my functional mobility (i.e. ability to complete normal activities like food shopping, going to the chemist or doctors is severely restricted).

I know you probably cannot comment but do you think it's best to just tick 'No change'?

I'm afraid we can't tell members what to write on their forms, it's a decision that you have to make.

It should but we have still seen members report that they had to have a face to face.
