You can be awarded PIP without any evidence although I would not recommend trying.

It will require you to be more detailed in explaining your problems with completing the PIP activities.

As Gordon has said, it is possible. When I made my claim I submitted no evidence, just the PIP2 form. I then had to rely on proving my case at f2f assesment which I was able to do.
In hindsight and with the knowledge I now have, I would advise everyone to submit all the relevant evidence possible at the start of their claim to present the best case in support of the descriptors and criteria that you think apply .
