PIP REVIEW FORM was created by lexus

I WAS AWARDED PIP ENHANCED RATE FOR BOTH CARE AND MOBILITY LAST YEAR UNTIL 2018, AFTER BEING TRANSFERRED FROM DLA AND A 2-HOUR LONG HOME MEDICAL ASSESSMENT BY A CAPITA PERSONNEL.

I'VE JUST RECEIVED A 20-PAGE REVIEW FORM. DO I TREAT IT AS A NEW CLAIM AND REPEAT ALL THE RESPONSES I GAVE ON THE ORIGINAL FORM, OR DO I TICK BOXES TO SAY NOTHING HAS CHANGED AND, WOULD THAT TRIGGER ANOTHER ANXIETY-INDUCED MEDICAL ASSESSMENT?

PLEASE HELP; MY ANXIETY LEVEL AND BLOOD PRESSURE ARE VERY HIGH AT THE MOMENT.
