GP Notes for DLA to PIP transfer was created by pollenpath
Hi Mods,

I received my notification letter about transferring to PIP and the deadline for me to call is the end of this week. I've managed to get a GP appointment for tomorrow and have just printed off the PIP GP Notes2 form. Although my GP is fairly sympathetic, he's also busy and won't have a great deal of insight into how my conditions (clinical depression & anxiety, PTSD, OCD on the mental health side, and migraine, osteoporosis and fibromyalgia on the physical) affect me in terms of the PIP descriptors. So I wanted to ask you, if I complete the GP Notes by ticking the applicable boxes and quickly take him through the form, would that be an acceptable way to proceed? He mightn't ultimately be willing to sign anything, I won't know until tomorrow, but as he's been supportive in the last couple of years I have a feeling he'll help here if he can.

Do you know if other members have essentially completed this form themselves before seeing their GP, and submitted it as evidence once their GP has signed it? I'm just aware there's so little time to ask of these people, as well as the risk that they won't have a complete enough picture to paint without my drawing attention to the finer details.

Many thanks,
pollenpath

You do not need any medical evidence to make the initial call to start your PIP claim, but you do need to make the call before the deadline or your CLA payments will be suspended, our PIP Claim guide has a list of the questions you will be asked on the call.

Thanks Gordon, yes I realise it's vital I make the call before the deadline and will make sure I do. I've delayed doing so before now because I've been particularly unwell recently and wanted to give myself some recovery time before facing the PIP2 forms and the timescale for completing and returning them

I asked about the GP Notes because it can be very difficult to get appointments these days and I had hoped to use tomorrow's appointment to go through the GP Notes2 form I downloaded from the members guides section.

Thanks again,
pollenpath.

Sorry, I thought from your post that you thought you needed medical evidence to make the call :)

Hello again :)

A bit concerned this evening that almost two weeks after making the initial call to claim PIP because my DLA is due to end, I haven't received the 'How your disability affects you' form. The call seemed to go well and I had a text message the following morning to say that they were sending the form, but nothing has arrived. I don't have problems with my post as a rule, I'm on good terms with my postie and live alone. During the call, the interviewer said I should call them if I didn't receive anything within a fortnight, which I'll do tomorrow. But will this affect my claim for PIP? I'm wondering if they'll suspect I've tried to delay the process by pretending the forms haven't arrived, or am I being paranoid?! These days it's so hard to know how the DWP will deal with individual cases. Do you have any insight into how this is likely to impact on my claim?

Thanks in advance,
pollenpath

It's unusual, but PIP2's do go missing in the post, they won't think that you are trying to pull a fast one.

Call them tomorrow and report the form missing, they will send a second copy ask the return date to be reset.
