Being in the run down state that its in, the GP system cannot be trusted to allow me to see any of my doctors by choice, it difficult enough to get an appointment. The GP notes to download are like a DWP form and I cannot see my GPs being keen to fill them in for me. Some of them have never met me ! Has anyone had any experience of their request being refused or had any positive response ? I realise each GP will have their own view on being asked to tick the boxes for a benefit claim.

We used the GP notes document for my wifes appeal. Not only was our GP happy to endorse the doc. she also kept a copy for use with other claimants that asked for her input

Thank you so much Peter/David! (My) Legal Advice Centre asked my GP for "evidence" a week ago but, unfortunately, it went to a new one who gave the simple summary info the DWP "advises" which the Advisor said was useless and Appeal would definitely fail.

However, after reading the template here, had the courage to go again and see the senior GP and now have a double appointment to see her + will take the B+W template.

This is/was terrifying + I was about to ask whether anyone had used/had any success with the letter as, like you, I felt it might be really onerous for them to fill in.

However, now feel much better re tomorrow - will report back on response but crossed fingers she'll have the same view as yours!!!
