What could the reasons be for not being able to plan and follow
a journey other than pysiological distress ?

The primary reasons are cognitive or sensory problems.
You may be able to argue pain, pain associated with standing and walking will almost certainly be considered under the Moving Around activity only. For other pain to count then it must impact on your ability to navigate the route, so the pain must either directly affect your cognitive capability or anxiety resulting from the pain must affect you cognitive abilities.

I suffer with CFS too and have difficulties with concentration, would this
be grounds for scoring points ?

You have nothing to lose by making the argument but you will need to explain your problems in detail, the DWP see cognitive problems as being the result of learning difficulties or brain injury and it can be difficult to get them to expand their view on this.
