GP just needs written request for evidence

Hi
I just asked my GP if he would write medical evidence in support of my claim form for ESA and they said they need something in writing that I can print off, to demonstrate to him that medical evidence is needed.
Please could you, a) either link me to something I can print off or b) reply on here to say written evidence is required so I can hand it to him, otherwise he may not do letter.

I'm not sure our words will be sufficient but if your GP is waiting for Maximus to contact them, then at this stage it is extremely unlikely that they will., they usually only seek evidence where there is a possibility of a Paper Based Assessment and this is looked at just after your ESA50 is received.

A claim without up to date medical evidence can be severely weakened and whilst it can be provided as part of an MR or appeal, we regularly see members conditions significantly worsened by having to challenge an adverse Decision.

Thanks for your reply but, I was unclear. I have still got my esa50.
My GP is not waiting for maximus to contact them. I have not submitted the esa50 and was wanting my GP to write a letter so I could send in with esa50.
My GP said before he writes a letter, he needs something in writing to say that evidence is needed to go in with the esa50. He cannot just write an evidence letter because I asked him to. Sorry for the confusion, is there anything I can print off to say it's best that medical evidence is submitted with the esa50?

No there isn't and the ESA50 actually advises that you should not seek new information.

I'm afraid this is down to your GP, they do not actually require anything to say that they need to write a letter or a report, it is entirely up to them whether they do this or not, it may be that they are simply dodging having to say "no" to your request by requiring a letter that they know you cannot source.

All I can suggest is that you push back, even though you have not submitted your ESA50 the likelihood is that Maximus will not contact them for further information, but even if they do then it will be Maximus that decides what questions are asked, which almost certainly will not be to your advantage.

Exactly what i didn't want to here.

I'm due a reassessment for ESA in October and a DLA to PIP assessment could happen at any time.I've got a condition that leaves me isolated and housebound most of the time.I'm on prescribed medication but i haven't seen any specialists for many years other than my GP..

My highly supportive old gp retired and the new one has a strict policy of only providing evidence for benefit claims when it's requested by the DWP.

I'm genuinely terrified by the process and the DWP were informed before that it left me suicidal with a serious worsening of my mental state. I honestly don't think i could cope with a face to face and i don't want anyone on behalf of the DWP coming into my home.

I've had this condition for many years now and the DWP know how bad i am.And they've got evidence of the diagnosis that was made around 20 years ago..My flatmate and family members could provide evidence -the latter that i can't even see them -but i'm told the DWP discards that because they think it's biased .

I'm feeling really anxious about this because i simply can't cope with the stress of it.And i was hoping that given all the info the DWP have on me and given the last prognosis on me was that it was unlikely i'd return to work i was hoping i'd be one of those exempt from reassessments from now on.However if i am summoned and can't provide any new medical evidence and i'm penalized for that then i know that's going to tip me over the edge.

Hi Gordon

Thank you, I will take in the ESA50 with me when I see my GP, or a copy of that page, as I dont want to lose it.
I'll try to look for something online like mental health websites to see if there is anything that says it is best to supply medical evidence along with the ESA50.
