Hello,

I've had my transfer letter. Already there have been difficulties, I can't use phone and my initial transfer was cancelled because I couldn't cope with telephone. The PIP1 I filled in instead was refused and they demanded a call within 48 hours which went very badly. I then managed to get someone from social services to call, luckily within the period given to call so still got my DLA this month.

The form took two weeks to arrive, it cannot come to me nor can any other letters or results as I'm being made homeless (another problem).

I've two twenty minute appointments to do the pip form. Typing answers isn't an option as they don't have a printer. I can't write the form and they're insisting I just answer in a sentence and tick box and go to medical and everything will be fine.

But the medical centre is now two counties away. Over eighty miles. I cannot get any evidence as medical services are a similar distance away and I can't travel. My GP will no longer provide any letters under any circumstances and social services said if I had severe depression I'd be on antidepressants but I can't take them so they won't support either.

There's no CAB or similar here. The majority of claims fail first time and at appeal due to no help. As good as the guides here are I cannot read them (severe depression and anxiety) and worker refuses to put any prepared answers anyway.

I can't get to medical or even have one at home and I'm terrified. Social services told me if genuine I'd get pip with no medical but I know that's not true. I didn't get ESA because DWP demanded medical and ATOS refused so only income is income support premium which stops if I don't get high rate pip.

I live in a county with the lowest number of unemployed and disabled people according to DWP - because of no help.

I've less than two weeks to get the form back by the date given. Worker said there's no rush as dates a guideline and claim I've an extension - letter today said no extension, if it's not back then DLA (and everything else I get) will stop.

Homeless accommodation here isn't paid for by housing benefit/LHA and I'll be refused a place if in arrears with no money (I pay weekly next DLA payment day after date on form likewise LHA).

I'm sorry this is long, I can't sleep and am having massive problems coping.

My GP yelled at me for bring silly for worrying.

To try and deal with the issues in your claim.

It is possible to have an extension for returning the PIP2 form, but it has to be requested, do you know if anybody has done this? Even if an extension has been granted it would not show on the reminder letter that is sent out by the DWP!

Have you notified the Assessment Provider of where you are currently living? It may well be that there is a centre closer to your current locale than the one where you used to live.

As to evidence, you can request a copy of your GP records.

Thank you for the reply. Apologies for the long rambling post, I hadn't realised.

The social worker asked for an extension and told me there's no problem at all, though they also think everyone gets pip so I wasn't sure, they also are under the impression it can just keep being extended.

There is no closer provider as the assessment centre in my county has been closed altogether, I didn't say but I'm still in the same county and town.

I asked for my medical records but was refused by surgery. They said they don't do that.

My DLA stopped for the time it took for me to get the transfer letter and for the call to be made - is that common? I'm worried it'll stop again! Especially as there's risk of hospitalisation.

One two week extension is almost always given, a second can be given but requires a Decision Maker to confirm it, so your advisor needs to ask early if they need a second one. A third extension is possible, but unlikely.

Tell your GP surgery that you have a legal right to the information, the only exception to this is if your records are make as "harmful", but if this is the case then it should have been explained to you. The links that I provided in my previous post explain how to make a formal request for your records.

Your DLA was suspended because you were late making the claim. Providing you complete the remaining requirements of the process there should be no further impact on your DLA until a Decision is made. However, if you are in hospital for more than 28 days then you must notify the DWP and your DLA will be suspended until you come back out of hospital.

Thankyou for the further help.

The GP surgery did mention it being harmful me having records. I'm guessing because of mention of my mental health issues.

I hadn't realised the DLA would stop as the call was made within the time given on the letter to make contact - I had until 5th September. Glad I didn't wait until then now!

Hopefully the form will be done by the new date. The plan is to put the most vital information on the form literally one or two bullet points and then attach typed notes and refer to them. I believe this is acceptable. There's no way all my needs can be fitted onto the form in the boxes they've provided. I'm doing a diary of needs each day to help me do the form.
