hi please help i am completing pip form its dla to pip transfer issue is i cant find any examples of filled in pip forms anywhere and also just changed my gp its in process what shall i putbon form

Although we don't actually have a sample form, our PIP guide does have examples of how you can answer, along with explanations of the descriptors you will be measured against.

I would give the name of your new GP as your records will be recalled from your old one - members might have different opinions for you! In practice, GPs are contacted in only around 25% of claims, so you should make sure you include any information you want the assessor to see.

May I suggest that you bookmark/favourite this on your web browser now so that you can find it easily in future? This will allow you to return with further questions or comments about your PIP without having to start a new topic each time. We ask members to keep everything relating to the same claim in one topic as it helps us enormously - and I hope you will find it useful too :)

If it helps - I did a similar thing, changed GP just before applying. I am about to send the "How your disability affects you form", so I can't say how this will turn out, but I put my current (new) GP on the form and asked them to write a supporting letter, saying it would be fine to consult with my old GP and of course my records (both GPs are at the same practice). I will attach this letter as supporting evidence. Obviously it's a bit thin on the ground because the new GP hadn't got to know me very well, but hopefully it will be OK.

gosh just completed the pip form absolutely horrendous given me s migraine guys wanted to ask i have 1 week worth of diary will that suffice as for some reason i cant find more on laptop

More is usually better, but a week should be sufficient.
