Unless I'm cracking up and missing something here, the DWP should put a sample copy of the PIP1003 form on their site - but I don't suppose they'd listen to me!
So there are some cosmetic changes to the form and the first couple of pages have slightly different text, but the actual questions on the new form are the same as in the guide so they should not be having any problems completing the form.
