PIP GP notes. PIP Renewal Guides

Help! I am trying to find the PIP GP notes to download and any PIP renewal guides. I've tried searching and can't locate either. Where can I find these?

May I suggest that you bookmark/favourite this on your web browser now so that you can find it easily in future? This will allow you to return with further questions or comments about your PIP without having to start a new topic each time. We ask members to keep everything relating to the same claim in one topic as it helps us enormously - and I hope you will find it useful too

I've just looked at the PIP GP notes. I think they will prove useful.
Just a helpful comment re: a typo on page 2 Preparing Food. It says "This activity is about my ability to prepare and cook a simple. (and then misses out the word MEAL).
Also if I post on the forum will my real name be visible, as it showed when you replied to me?

My full name is showing, how can I stop it?

These things happen. It's great you are producing a useful document. Will an amended one be available soon as would like to give to my GP for a PIP review?
I think I've changed my username on the forum but how can I omit my name it from the messages you've sent. Help.
