Hi there I have had a esa50 reassessment form sent to me. Have completed and filled in and today find out that my go has an e113 form to complete. I have severe and chronic lumbar arthritis along with arthritis in the neck as well as anxiety and depression. I never see the same Dr and am worried about this form being filled in and also as to why the dwp have requested it. Would it be worth me trying to get appt to see Dr to discuss?? Drs NEVER seem to have much time to discuss anything when I am there.

You should be able to request a copy of the ESA113 from your GP Practice, perhaps do this first so you actually know what has been said before you talk to your GP. Of course, if the form has not yet been completed then it may help your GP to discuss it with you but be aware some GPs may not want to do this.

Hi there wondering if I can get some help. I am so stressed and worried my go received E113 form and on the main QUESTIONS 're movement and getting on transport he has scribbled NO FORMAL ASSESSMENT DONE AND DON'T KNOW. Also I have had x rays to confirm arthritis of neck and spine but he makes hardly any mention. I never see the same gp and have seen him twice. I paid for a letter that just states medication and conditions on my medical notes. My previous gp was wonderful very helpful. This gp surgery just gets you in and out. Do you think this is going to make DWP call me in for face2face also is there anything else i can do. Thanks Ann

I'm afraid the GPs comments do make it more likely that you will have a F2F.

You can get a copy of your medical notes from your GP practice, these should contain any letters that have been sent to your GP by the Consultants that you have seen at your local hospitals.
