I have my dla-pip assessment next week at my home. Offered without request. I have asked which type of health professional will be doing it and their name. Capita tell me it’s male, but that’s it, refuse to tell me more. Previously from maximus for esa I’ve has a letter giving their name. Is this correct? Do I have a right to know who is coming to my home?
Also I’ve asked for a copy in advance of the form i have to sign re recording assessment. But they said they couldn’t provide it. Is there a link anywhere or a copy I can download?

I don't think it is an unreasonable request for the assessors name, after all you are being asked to allow a stranger into your home, however, it's possible that an actual assessor has not been allocated yet and that is why no name has been provided..
I would try calling again a day or two before the appointment.
I'm afraid I'm not aware of anywhere to download the form that they will use for the recording.
Just to confirm, you have permission to record the assessment and are able to make two simultaneous recording directly to tape or CD. Remember that digital recording is not allowed.

Thank you for that. Yes I bought 2 cheap cassette recorders and cassette tapes in Argos in Lancaster, seemed the cheapest option. The till lady told me they sell like hots cakes and people always buy 2. She said she couldnt understand it with all the tech available these days !
