Hi,
I've just received my PIP pack after telephone conversation, I am transferring from indefinite DLA Medium Care and Higher Mobility to PIP.

I was wondering if one can complete the form using printed annexes only referring to the annexes on the form spaces. As I find handwriting almost impossible or at the very least very painful and slow. I also find due to my pain killers my short term memory and concentration levels are rubbish.

I am proposing to place a marker in the form box such as "see sheet 1a" and then on a printed paper document that I have typed have Sheet 1a and the reference to the question number etc and type my answer.

I didn't realise how complicated this sounds so I apologise

I'd appreciate your advice..

It is perfectly OK for you to type your answers and attach them as extra pages to the PIP2, just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.

Attach the pages to the back of the PIP2 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.
