I wanted to know how can I get a template for my GP to write a supporting letter
I’ve already gave my specialist letter but they have now asked for a factual report from my GP is there any advice you can give me I’ve paid my subscription but I can’t seem to download or view any templates

Have you looked at the GP Notes document in the PIP section.

Thanks Gordan

The pip people asked me for a factual report so I asked my GP who is in support of my claim so is my neurophysiology along with friends and family who have supported me since my accident they have all wrote supporting letters explaining how they help me with everyday life like house chores shopping bill paying Hoapital appointments and counciling appointments. In my letter from my neurophysiology it states I’m suffering with anxiety memory problems sleep and concentration and low mood it also states I can not do every day tasks as this was asked in the pip forms
I’m have been under so much pressure and stress and distress it’s giving me suiicidal thoughts I’ve just had an operation to correct my breathing because my nose was also damaged in my accident I sustained a blood clot heavy bleed on my brain December 2015 along with fractured eye socket damaged nose and fractured jaw
I woke up to my head shaven and 57 stitches across my skull and memory of what happened I feel I’m being victimised by pip as my doctor and neurophysiology can’t understand why they want to come see me after my supporting letters
I just feel to weak to fight them for my rights although everyone around me said they will attend any appeals court or anything else as they all have said keep going I just feel like I’m to stressed to continue and scared of the meeting as they have already made me feel I’m lying or inventing this it’s all taking a total

I'm afraid I have never heard of the DWP asking the claimant to source a GP report.
