Format of PIP2 Form was created by Daffodil99
Hi, I am new to this forum so please bear with me.
I am still waiting for the PIP2 form to arrive by post but I am unsure of the current format. I think the DWP made changes to the form around May - is there still a yes/no/sometimes option? Is there an additional text box immediately after each question? Or does anyone know where I can find a copy of the current form being used? I have asked the CAB but they didn't have one and could not advise. I am trying to get prepared but finding it difficult not knowing if all additional information has to be included at the end. Hope that makes sense. Many thanks.

There have been some cosmetic changes to the form, layout and some of the unimportant wording has been changed but the questions asked are the same as when the form was originally created.
The current form is not available in the Public Domain.

Within the cosmetic changes, do you know if the additional text box after each question has been removed?

Given the updated form has been available for some time, I am guessing that the DWP do not intend to make it available to the public.
