Hi everyone, its my first time here 
I have an appointment for a assessment. I have been on ESA for a couple of years now. Can anyone tell me if I need to fill in any forms beforehand to send in or to take with me?

If you have not received an ESA50 then have a look at the following.
Invited to a face to face assessment but no ESA50 sent
