Now I'm really unsure about it. I don't know what to do.

So better to say 8 months out of the last 12 I have been housebound due to crippling anxiety and depression. This is aggravated by my constant chronic pain. I am even more reluctant to leave the house due to my episodes of collapsing which the consultant seems to thing stems from my severe anxiety. On the 4 months I have been able to leave the house it requires huge effort on my partners side in trying to motivate and calm me enough to leave the house. I can't go places I'm unfamiliar with at all and wouldn't be able to use public transport without my partner due to the risk of bad things happening and my crippling anxiety when there are lots of people around as I wouldn't know how I would react alone during a panic attack. I also need her incase I collapsed, she has had to carry me before when the episode of severe weakness occurred. And she would also be needed incase of a bad hypo episode when I'd need someone with me to ensure I didn't go into a coma.

Is the above roughly on the right lines?

Yes, but you need to make it very clear that any collapse is due to your anxiety and not a physical problem, or you risk it being ignored and I would not use the word "anxiety" if you can avoid doing so, it suggests a more trivial condition.

Generalised anxiety disorder and agoraphobia, are they better terms to use then?

That's my diagnosis so I guess I need to use the correct terms instead of anxiety.

Yes these would be better, as a general comment always try and reflect the language used in the Descriptors back to those assessing you..
