I have my WCA this coming Friday as I have been receiving assessment rate since August after a successful MR. I was sent the ESA50 form to return and complete in August, however I became seriously ill with sepsis and was admitted to hospital (still in recovery after being discharged.) As such, I was unable to return the form and my husband spoke to the assessment centre who accepted this explanation for the lack of form.
My query is do I need to complete an ESA50 form and take it with me to my assessment despite never sending one in previously? My assessment was rescheduled twice due to my sepsis and an unavoidable childcare issue so I am aware they may not reschedule again if I contact them about my non submission of the form.
It is very unusual for a new claim for ESA not to have an ESA50 issued as those assessing you will have no knowledge of your problems, so yes I would either complete an ESA50 or write a letter in the same format explaining how you meet the ESA criteria.
You should give the assessor a copy at the appointment and ask them to read it before they continue with the interview, I would also send a copy to the DWP office dealing with your claim, the address will be on any letters that you have from them.
