I spent a lot of time going through your notes and managed to get standard rate DLA for me and my wife and no mobility.
I put in a manadotory reconsideration I have not yet had mine back but my wife got no change. The grounds I used were how bad the assessment was and the lies. They have said there is no evidence of this. They ignored my wife's cognitive issues totally. She also had very little evidence of anything as she has been diagnosed for 5 years and because I was okay and on a good salary never claimed. Now as things have changed for me I have had to put her through this process.
Is it worth an appeal?
I am now worring as I have my ESA assesment on Tuesday and my wife will have to have one eventually and we both cannot work and I am worried we will not get into the support group after the PIP result. :( :( :(

I'm afraid I can't tell you whether it is worth your appealing or not.

The key issue is whether you can reasonable score the extra points needed for a higher award. If you think that you can then consider an appeal, if you can't then don't.

Remember you need to be able to complete the activities reliably and on the majority of days not to score, so would this make a difference?

If you decide to appeal then the PIP Appeal guide has links to the SSCS1 form that you will need to complete and the Submissions guide has a complete example that you can use to help you fill it in, you do not need to provide detailed reasons at this stage for your request.

Once your appeal is accepted the DWP will prepare their submission to the hearing and send you a copy, whilst you should wait until you receive this to finish and submit your own, you should start to prepare your own case, an increased award will only be made by your showing that you score the extra points for one, so I would start by going back through your PIP2, the assessment report and the PIP Claim guide to see how you might show this is the case.
