hi, if youve been sent a pip renewal form asking if anything has changed and you dont have a copy of your original form because an advocate filled it in while you were mentally ill and have no recall of what was put, what on earth do you put?

Can you contact the advocate for a copy of the form?
If not then all I can suggest is you tick "no change" where still applicable and where your limitations have changed, provide a detailed answer of how your are now effected.
