Where to send decision maker copies of evidence was created by Tranquility125
Apologies for asking this as it must have been asked before but I can't find it.
If I want to send a copy of my evidence to the decision maker as well as returning my form/evidence to Capita what address do I use for the decision maker? I only have the mail handling address at Wolverhampton.
Also do I send a photocopy of my completed form to the decision maker as well as my evidence?

The address will be on any letter that you have had from the DWP in regard that benefit. All DWP post goes to Wolverhampton and then is forwarded to the relevant office.
It's up to you whether you send the claim form directly to the DWP, there used to be a problem with forms going missing but that almost never happens now, so there really is no need to do it now unless you are still concerned.
