Hi. Looking for advice on the most useful evidence people have used to support care needs. I could get a supporting letter from GP but not sure if this is strong enough evidence as he will be summarising information I have discussed, rather than personal knowledge of care needs. I could ask a relative to write a letter but how detailed does this have to be? Also as I only have these two options do I include one or both with the claim form or even just wait until they ask for further evidence and just fill the form in without sending the evidence. I don't want to use all my evidence in case I need to appeal as I don't have any more evidence. Thanks

I would be careful of using the phrase "care needs" this is a term used for DLA, PIP is a functional assessment and has very different criteria from DLA.

If you are looking for something to provide structure to a GP letter then there is a GP Notes document in the PIP area that may be of help.

A friend or relative can make a statement by writing a letter of support, it needs their name and address, date, relation to you and how often they see you.

The letter needs to explain anything that they do for you and any observations that they have about your problems with completing the PIP activities.

Lastly they need to make a statement that the information contained in the letter is true to the best of their knowledge and obviously sign it.

There is absolutely no value in holding evidence back, this is about getting the right award as quickly as possible.
