Request home assesment, when is the best time?

Pip2 form
Once I get my Pip2 form should I request a home assessment on my form or leave it until they contact me regarding my assessment? as going to a centre would be physically and mentally impossible. I could not go but I'm worried in case they will use this against me as if they know I cant do it even if it went to a tribunal I wouldn't be able to make that either so I'm scared of losing because of this

You won't get a home assessment without your GP writing to the Assessment Provider explaining why you cannot attend at their offices, the letter must be unambiguous about your abilities so phrases such as might have problems or could lead to a deterioration in the claimants conditions will not be helpful.

If the letter is available before you return your PIP2 then include, but don't delay returning the form to wait for it.
