ESA Contribution based or means tested

I've been told by DWP on the phone that I have been awarded ESA Contribution based for another 3 yes in support group, but that I might be entitled higher rate for SDP and backdated to 2014. They sent me the form to complete.
As I get occupational and private pensions and care from Social services under the Direct payments scheme (this pays for agency carers and 2 personal assistants) I feel this might stop me getting SDP.
If I submit the form for means testing ESA and do not qualify, will I still get the contribution based ESA, or do I risk losing that?Thanks.
