You only need to provide information for the activities that you are challenging the points on.

In dad's MR decision letter there is no explantion for any of the scores. Should i have an explantion of the mandatory reconsideration scores? I have the original assessors report and reasons for his original decision but not the mandatory recosideration.

For ESA there is an LT54 form that is compled by the DMs, but I'm unsure whether this is used for PIP or not, a;; I can suggest is that you ask for any documents produced by the second DM.

Can I leave the section about representation blank on the appeal form? I'm still trying to get him an advocat but so far I've had no luck getting any help for him where we live.
